With that I have been able to achieve  better outcomes and have presented the data at national meetings and  in Diabetes Research and Clinical Practice. All those details are in my website.
I am looking to collaborate with software developers from EHR companies to  fascilitate using this model by the team of four, ie the physician, their staff, the patient and supportive family members. 
Achieving better outcomes using physicianfriendly and patient friendly software could allow meaningful  risk reduction of expensive complications from chronic diseases.
I was wondering if I could discuss this further with your product development team.
